APPLICANT AND PARENT CONSENT

I understand that the staff, other volunteers and families of Hope Bible Church Niagara are counting on me to fulfill the following commitment. I have discussed these with my parents/guardians. *

- I will arrive one hour before the start of service when I'm scheduled to serve.
- I will allow adults to handle washroom breaks and check-in/check-out procedures.
- I will stay in the class and engage children in activities as I am able.
- I will participate in both classroom and large group activities.
- I will model respect of authority for the children by listening to the storytellers and submitting to adult leadership.
- I will be flexible and move to other classrooms as needed.
- I will call or email appropriate ministry staff if I cannot serve when scheduled.
- I will leave all discipline issues to the lead worker/captain.
- I will agree to stay off my cell phone when I am serving and be engaged in my role.
